Release and Acknowledgement of Risk

In consideration of the permission by Lake Hill Hunt Club (“Lake Hill”) for the Greater Columbus Pheasants Forever Youth Hunt event on the premises of Lake Hill on October 17, 2020, which will include the use by minors of firearms for hunting and shooting,  I hereby release and forever discharge from any and all liability Lake Hill, its members, the volunteer members of the Greater Columbus Pheasants Forever chapter, as well as any representatives of the ODNR/Ohio Fish and Wildlife, and the agents, owners, officers, and all other persons or entities acting in any capacity on behalf of the aforementioned groups (“Groups”).

I understand my child and I are guests of Lake Hill and minor children will be using loaded firearms. I acknowledge I am aware of the risks associated with the use of firearms and archery equipment, including potentially fatal accidents, and I understand my child and the other participants are fully at risk when these activities are undertaken. I expressly agree and promise to accept and assume all risks existing when my child handles loaded firearms. Further, I agree to indemnify and hold harmless the Groups from any claims that may be asserted against them as a result of my child’s use of a loaded firearm.  I agree that in the event of litigation as a result of my child’s participation in the event, it will be controlled by Ohio law and Ohio courts have exclusive jurisdiction.

Additionally, the Greater Columbus Pheasants Forever chapter (“GCPF”) has put in place preventative measures to reduce the spread of COVID-19; however, GCPF cannot guarantee you will not become infected with COVID-19. Further, attending this outing could even increase your risk of contracting the virus. By signing this document, you are acknowledging the contagious nature of COVID-19 and voluntarily assume the risk of exposure or infection that could result.

I hereby release and forever discharge from any and all liability the Groups, their members, volunteers, representatives, agents, officers and any acting in any capacity on behalf of Groups. I expressly agree and promise to accept and assume all risks existing as a result of my participation in a Youth Hunt during a worldwide pandemic. Further, I agree to indemnify and hold harmless Groups from any claims that may be asserted against them as a result of my participation.

By signing this document, I acknowledge that I have had sufficient opportunity to read it in its entirety, and that I have read and understand it and agree to be bound by its terms.

The first 10 registrations received via email or mail by October 10, 2020 will be accepted.  All other children will be placed on a waiting list and will be notified of a cancellation.
